HMO Appointment Guide
Follow these simple steps to make your HMO consultation and appointment process easy and hassle-free. From verifying your HMO coverage to completing the necessary requirements, our step-by-step guide will help you navigate the process with confidence and ensure a smooth and convenient healthcare experience.
Step 1
Scan the QR CODE to register for the online approval.

Step 2
Fill out the form with patient’s details, then click submit. You will see confirmation that your response has been received.
Sagutan lahat ng detalye ng pasyente at i-click and submit. May mababasa kayong kumpirmasyon na natanggap na ang inyong detalye.
Step 3
The turnaround time to process the approval is 1-2 days depending on your HMO provider. For procedures with preparation, the approval process should be done at least one day before the test.
Ang pagproseso ng inyong aprubal ay 1 hanggang 2 araw depende sa inyong HMO provider. Para sa mga eksaminasyong may paghahanda, ang proseso ng aprubal ay dapat gawin isang araw bago ang eksaminasyon.
Step 4
Within 1 to 2 days, our HMO staff will call or text your registered number for feedback and other instructions.
Sa loob ng isa hanggang 2 araw, tatawagan or ite-text kayo ng aming HMO staff sa inyong inirehistrong numero para sa katugunan at iba pang tagubilin.
Step 5
Follow the instructions and proceed to HMO Section to get your approval, and sign the needed forms. Present your HMO card and ID for validation.
Sundin ang tagubilin at pumunta sa HMO Section upang kunin ang inyong aprubal at pirmahan ang mga kailangan papeles. Ipakita ang HMO card at ID para sa pagpapatunay.
Step 6
Follow the instructions and proceed to HMO Section to get your approval, and sign the needed forms. Present your HMO card and ID for validation.
Sundin ang tagubilin at pumunta sa HMO Section upang kunin ang inyong aprubal at pirmahan ang mga kailangan papeles. Ipakita ang HMO card at ID para sa pagpapatunay.
Step 7
Proceed to the respective department (OPD/Laboratory/Imaging), and present the form with approval to get the procedure done.
Magpunta sa naayong departamento at ipakita ang form na may approval para maisagawa ang mga eksaminasyon.
