Hospital Admission Requirements
Prepare the necessary documents and information to ensure a smooth and hassle-free admission process at Antipolo Doctors Hospital.
Personal Identification
- Valid ID (e.g., government-issued ID, Senior citizen ID, or School ID for minors)
Doctor’s Admission Order
- Referral or admission slip from your attending physician(if non-emergency)
PhilHealth Documents
- PhilHealth ID
- Member Data Record(MDR)
- Updated PhilHealth Contribution receipts(if needed)
- Signed PhilHealth Forms(CF1, CF2, etc.)
Health Insurance / HMO Card(if applicalbe)
HMO Card or LOA(Letter of Autorization)
Valid ID matching the insurance record
Admission Process for HMO Card Holder
We welcome Health Maintenance Organization (HMO) card holders to our facility and are committed to providing the best care during your stay. Please take note of the following:
Present your HMO Card Upon Admission
- To ensure that your coverage is applied correctly, please present your valid HMO card upon admission to our Admission Section. This will help us verify your eligibility and coordinate with your provider.
Coordination and Approval
- For faster processing, presenting a Letter of Authorization (LOA) upon arrival helps speed up your admission and discharge process. If you don’t have one, our staff will gladly assist in coordinating with your HMO provider.
Waiting Time for LOA (Letter of Authorization)
- Please note that the waiting time for your LOA (Letter of Authorization) may vary depending on your HMO provider. On average, please allow 4 hours for the processing of your LOA. We appreciate your patience and understanding as we work to ensure timely approval from your HMO.
Coverage Details
- Please check your HMO policy for coverage limits and out-of-pocket expenses. Be familiar with the details of your HMO coverage, including eligible services, limits, and exclusions. This will help avoid delays and ensure smooth admission process.
Out of Packet Expenses
- If your HMO does not cover the full cost of your hospitalization, the remaining balance will be charged to you directly and must be paid before discharge. Our billing staff will inform you of any amounts not covered by your HMO.
PhilHealth and HMO Claims
- PhilHealth benefits will be processed separately, and any applicable deductions will be applied to your final bill after HMO benefits are deducted.
Discharge and Final Billing
- Once your doctor’s discharge order has been given, the billing team will finalize your account, including any remaining charges or professional fees (PF). All bills must be settled before discharge.

Note: *During a pandemic or such unusual situation, health protocols mandated by the Philippine Department of Health and guidelines issued by the ADH Infectious Medicine Unit will be strictly implemented.
